I authorize R.F. Owens Co., Inc. d/b/a Trucchi’s Supermarkets (referred to as Trucchi’s Supermarkets) to investigate all statements contained in this application, including records of any former employer, doctors, hospitals, police departments, and other services concerning me, and authorize such sources (and Trucchi’s Supermarkets hereafter) to release such information without liability for any damage incurred in giving it.
